The monument was moved to Tawstock from St Ive, near Callington, Cornwall in 1924.

Arranged in three panels, the top with a shield of ten quarterings, the middle has shields with fruit & cherubs; the figures in the third row are of bearded John Wrey in front of his mother in ruff & gown; the figure on the right is Blanche's husband John Wrey, d.1576
